Cube Value Application Process

Step 1: Fill up the printed copy Cube Value Partner System Application Form that you download and print out from the link provided below:

Step 2: Sign and stamp your company chop.

Step 3: Please attach a photocopy of your I.C. and Business Registration Certificate (form A & D for Enterprise or form 9 & 49 for Sdn Bhd)

Step 4: Scan & e-mail, fax or post your completed application form and relevant documents to:

CubeTech Asia Sdn Bhd will send you your account ID and a random password which you must use the first time you log in.
You may login at http://www.doa.com.my/p_login.aspx. You may then change the password after logging in the first time.
